They are referring to the document that came with your EAD attached.
I think it says on there something like: "For faster processing, attach this the next time you apply..."
It has three bar codes on the paper

For the addresses, we only list from last renewal, correct? That's what I did last time without issue, but I figured I would double check

Is there any disadvantage to sending vis FedEx UPS DHL vs USPS? I noticed they go to different addresses. I've always sent via USPS previously but they are closed on Monday so I thought I would try different one.

Pg2 Right side bottom
"For Renewal Requests: List only the addresses where you resided since you submitted your last Form I-821D that was approved"

BTW uscis site still says for people expiring between Sept 5th and March 5th, what do you guys think about that? I'm surprised it wasn't changed.

Do you need color copies of current EAD? Or does black & white work?

Am I missing anything? If I read everything correctly this morning... we just need to include I-94 (those that at least have it) and old EAD Card.... Nothing else no?

You don't need to send I-94 for a renewal request.
