Help I've lost my work auto card
 

I've somehow misplaced my work authorization card. I've look everywhere to no avail. How do I go about to getting a new one? Does anyone what process one must take? Help, please.

You would have to apply for a new one. Print out the I-765 Application. Print a copy of your DACA Approval Letter and your Employment Authorization Approval Letter and send it with it.

It'll cost you $380 to get a replacement EAD.

EDIT: You can't E-File either. You have to send it to the corresponding Lockbox.

Many people say it's $465 and that their checks or money orders of $380 were returned to them.

So they know you're who you say you are and didn't just give your EAD to your Amigo who kind of looks like you while you get another one.

I am surprise they don't ask for a police report, but then again - having to do another biometric ensures you are really you

Would I need to fill out the work sheet again? Or just the application?

You should submit any form that is associated with the work permit application. That would include the worksheet where you put in your assets, etc.
