Someone I know came up with this idea in order to get our stories to the sentors in a short, easy to read format. He wants as many of us to fill this out as possible. PLease fill in the blanks (---------) . And either post it on this thread or PM me with the answers. The person I know will fax as many of these as possible to the sentors. If anyone has any comments or suggestions about it, please let me know as well.
