HELP with 765 WORKSHEET
 
If im a single mom and my daughter's dad gives me $350 every month for her do i add that money as my income as well?

I don't work because i don't have a SSN but i help my dad around a couple of days a week in his small business and he gives me around $200 every week. Do i put that as my income? or do i put that i don't work but that my father helps support me? He also helps me pay for my school (Classes and books)

I live alone with my daughter. I pay rent, bills,etc.

Yes, put that as your income (your baby daddy's money and the money your dad gives you every week).

For expenses, include what ever bills, rent, etc. you pay.

for assets, include value of any property, vehicles, stocks, and current balance on your checkings & savings account.

Then explain how you get your income (pretty much the same explanation you gave on here, and what expenses you pay).

If you want to submit any evidence on your income, expenses, and assets, it is completely up to you since is not required to do so at all.

I work of the books I can't lie about not working because I'm using bank statements as proof that I've been here should I say that I work but not full time. I can't use my bosses name on anything. Should I say i
work as an independent contractor or something?

Do not say you work as independent contractor because you would need a self-employment tax ID number known as EIN
