i-765 question
 

This might be the dumbest question ever, but I am having trouble figuring it out.
The first question of the form, where you enter your name. How do I enter it there? Is that where I enter my full name? because it says

1. Name (FAMILY NAMES IN CAPS) (First) (Middle)

this is what has me confused. Does it only want my first and middle name?

and another question about the seconds question.

I have a first name, middle name and 2 last name. I never use my second last name for anything.
so lets say my name was John William doe Ortiz.

I always use John William doe. For everything. Even though it says my two last name on passport. How do I add the other name to the other names question?

Family Name in caps would be your two last names in caps: DOE ORTIZ ( i may be wrong, because I only have 1 last name)

Then you type your first/middle name normally: John William

i have a question too in my i765.. on my point of entry if forgot to include the state, i only put LOS ANGELES no CALIFORNIA.. do you think this is acceptable? or do i have to put everything like LOS ANGLES CALIFORNIA.. thanks

Just have to wait an see... it depends on the person evaluating it, so hope for the best.

you really should have put California, but i'm sure its not a big deal, don't worry:)
