Biometrics form......?
 

For those of you who filled out a form when you went to your biometrics appointment, does it ask for parents or grandparents info? Whats on it?

If yours asked for that you're probably in the wrong place.

No it doesn't. Asks for your name, alien #, reciept #s, height, weight, eye color, hair color and bday. Most of it is what's already written on your notice, you just copy it.

They only ask First Name, Last Name, Names You've Used, Hair Color, Eye Color, Weight, Height, and Application Numbers if I remember correctly

no! you don't have to go with your mom and dad all you have to do is answer some questions like whats your address and were u from and that kind of stuff hope that helps!
