some questions regarding renewal process.
 
Hope all of you guys are doing well. I thought this website is the most informative website regarding renewal application and I am seeking suggestions for few questions I have pertaining to renewal process.

1. I-765WS form.
I have overall income just above $100,000 but some of it is off the book and some is on the book (bonuses etc). Does it matter what I put on it? Do I have to put what I will be putting on the tax return or just raw total income that I make? (I do not report cash income). Will the amount make any difference on the outcome or are they just trying to figure out statistics on DACA income?

2. I-765 form.
On query 10, is the Alien Registration number (A-number) 9 digit number you can find on your EAD card? If it is, do I write A-123456789 or just 123456789?
On query 11, regarding which USCIS office, my previous application has been approve in Vermont but which date should I put? The date of approval of EAD or the date of application sent?
On query 14, if I entered the US EWI, is NO LAWFUL STATUS appropriate?
On query 15, are we all TEMPORARY VISITOR TO WORK as our status?
Query 16, I put (c)(33). Is this a correct validation for our renewal application?

3. I-821D form
On travel information, I have never left US since I was brought here. Do I still have to fill it out? It seems like I might have to fill out 8-10, but I never renewed my passport for the last 20 years.

thanks guys in advance. DACA totally changed my life and I couldn't be happier at this point. Although I am not doing what I have dreamed of (being a doctor), DACA certainly changed many things in my that I am very thankful for.

2. 1-765 form
  • #10: Yup, the number on your EAD card under "USCIS #." Include the A, so it would be A-123456789.
  • #11: I put the receipt date but the date the application was sent works too (I don't remember exactly when I sent mine in but the receipt date looks very close). My reasoning is that the question says "Have you ever before APPLIED for employment authorization from USCIS?" So I think it would make sense to put the date you applied, which could be when you sent your stuff or when they received it (assuming the dates are similar).
  • #15: I believe we are just DACA.
  • #16: Yes (c)(33).

3. 1-821D forum
You don't have to fill out the travel part since you haven't left the US since your initial DACA application.
