c33 and IOE number and service center
 

I am filling out the renewal packet
Will be looking for my EAD approval letter later but on my card, the Card# is begins with IOE.
Which service center is this? Would the service center be indicated on the approval letter?

On the Work Authorization form: For the part which says:
Service Center:
Date: (Is this the "Valid from" date on our EAD card)?
and also, for the category, it still remains as c33. is that right?

Actually, checking on my email notification from USCIS, 2 years ago, the same email which had the routing information and receipt says it went to the Nebraska center, so I think Ill take that.

Category is c33 for daca.
Ioe is the ELIS- you can just write ioe

I put the date I applied for my last renewal since it's asking if you every applied for renewal. It's not very clear which date they want though.

I put the date of my card.

So it seems it doesn't matter

I never submitted the approval notices. In fact I don't think I received an approval notice for I-765 except the first one.

I made a mistake on this part and put the first time I applied for EAD back in 2004... do y'all think this will affect the process even though i sent pictures of my current EAD? I saw that the uscis has service contact number.. could this be used to make minor corrections?
