I don't think will affect your application. Were the police involved? Were you arrested? It sounds as if the school just handled it since you said you were suspended.

hmmm, I'm not sure. Did you work with a lawyer before when you received your stay of removal? If yes, your old lawyer will probably be willing to answer that without charge. My dad's lawyer answered...

Your last entry would also be your first entry if you never left since 1991. So don't leave it blank, but the date of your first entry.

That is a difficult situation if your passport picture is from when you were three. If there is no way to get your passport renewed with an updated picture, then I would definitely bring a birth...

He doesn't have to show proof of the day he entered without inspection into the US. He does have to show proof that he entered before his 16th birthday (as well as all the other qualifications)

You can write it out. If you are doing USPS express, you don't even have to write anything on the envelope. When you get to the post office they will give you this form to fill out which they stick...

He's still in high school? I assume he has started classes for Fall term? I would put Sept 2012...you could even put the exact date of today (or whatever day it is you are filling out the form).
