If you didn't come with a valid visa: entered without inspection
If you had a visa: put the visa you came with
(If not sure, ask your parents)

The transcript is the important one. I don't think USCIS would expect everyone to have saved high school diplomas. It's nice to have it, but as long as you have the transcripts, you should be fine.

You fill out the In Care part only if another household (or anyone that handles the mails before you + your family) lives at the same address. In general, it's left blank.

No need to write your name on the back. I don't remember seeing an instruction to write it (correct me if I'm wrong), and I didn't, had zero problem.
