With regards to your other name, if you have enough proofs under your legal name, I suggest you just use all that and list the name you go by on other names used. Simple is always better, and it will require less explanation.

If, on the other end you need to use documents containing your other name, then you should include an affidavit. Along with evidence that you are the same person.

In my case, as suggested by my lawyer, I included a notarized affidavit from myself and one of my parents explaining my different names. The affidavit is commonly known as "One and same person affidavit." You can find many examples online.
I basically explained the different names on my documents and how the name came about etc. And included evidences such as school ID, medical records showing my date of birth, etc. I only did this because I had no choice since a lot of my important documents were under my other name.
