yes, your birth certificate, EAD and SS5 form should be sufficient. when i went in today to apply i only took my ead and passport, but the lady said i have to show a birth certificate if i have it. now if i had said i don't have a birth certificate then they would accept my passport only. i looked at her for a few seconds like "did you just hear yourself? you're making no sense". but there were a lot of people there so i just drove back home and got my birth certificate.
about the 10 day wait, some people say they didn't have to wait, others say it's better to wait. i got my ead yesterday and went to the ss office this am. they gave me a receipt and said i should receive my card in 2 weeks.
