I been a manager for McDonald's, Jack in the Box, KFC and Del Taco in the past. I can honestly tell you not to worry about it. Just go to the interview, if you get hired, you will fill out some paper work (I-9) and provide a copy of your ID and social. They just use the application to know your basic info (name, phone number and experience).
