Apologies if this has been answered before, but can anyone help me on what to put for the "Date(s)" in I765's Question #11?

I've searched the web inside and out - some say approval/validity date, others say receipt date, some say notice date...

There is also the date that's provided on the receipt where our EAC was attached (the one w/ the barcode...)

It probably doesn't matter, but what date have you guys been providing?
