You can't get it from your old address? It looks like you'll have to get a duplicate license from the DMV and pay the associated fees along with it. You could try going into a DMV center and try to get a duplicate for free; explaining that they made the mistake and that they are responsible for sending it to the right address.
Otherwise, if you put the previous address, then there's not much you can do. You'll have to change the address on file and order a duplicate.
