That sounds like a terrible mix - up.
I don't have any experience in this but have you tried emailing the service center to which you've sent the forms? I've sent two emails to my service center (NSC) and they replied one time with some good information.
You can also write a letter to your service center (make sure it's certified!) explaining your situation. I've tried this one time but with no avail

(But I've heard of successes as well)
Also, I guess this might be out there but you can try to contact your congressman/congresswoman.
And if you want to reach out even further into the "if" section, I guess you can record your phone calls and keep a log of the agent ID #'s in case you need some proof later down the road for representatives providing you with incorrect answers.
