The affidavit of support is required for all green card apps. They will need to know that your spouse and/or someone else make enough to support you. Medical exam is also required, you'll need to visit a civil surgeon, they're basically everywhere you can search for them using USCIS civil surgeon locator. Lawyers typically will ask $1,000 or more for green card apps; there are some that offer for less, but a quality one is more than $1K (but not always). Research, make sure they're registered, etc.
