These only apply for those who had a re-entry permit* or a refugee travel document** before. Since in order to get a new one you must either attach the old one, or claim that it was lost/stolen/destroyed/etc. This question does not apply to applications for parole (including advance parole (I-512)).

* - (I-327) - Passport-like document issued to permanent residents seeking to be out of the country for over a year, but less than 2. Needed so they don't lose their permanent residency. This document may be sometimes used in lieu of a passport for international travel, and visas can be inserted into it, however this is really up to the country you're traveling to whether or not they accept it. Valid for 2 years (1 if you've spend the past 4 out of 5 years outside of US while being a permanent resident).

** - (I-571) Passport-like document issued to refugees/asylees and permanent residents who obtained their permanent residence via refugee/asylum processes, since generally by claiming persecution from a country you generally cannot get documents from them, and generally - you don't want to either. This document is generally accepted as a passport under international law. Valid for 1 year.
Wow can't believe I missed that one. Missed out on the "Refugee" word. Usually I am very good at filling these applications out. Thanks DEMISE brah! In that case I should not even mention my initial A/P in the official cover page that I will be sending. Keep it plain and simple.
