1. I recommend putting in the application ASAP since you never know how long it'll take for them to renew it. Also, with the recent news about the states threatening to sue Trump, you may want to get approved sooner than later. The downside is that if you get approved soon, then you will lose a few months off your EAD since the new approval period starts immediately.

2. Estimate as best as you can. I set up a spreadsheet where I listed all my income/expenses and got pretty close to a real figure. You may need this if they write back to you requesting more information as to how you came to the numbers.

3. Put the approval date of your previous employment authorization. Attach a photo copy of your current employment document and also the I-797(?) approval notice.

4. I'm not sure - I've included them both times to be safe. It's only $15 or so :/

5. "U.S. Department of Homeland Security" (write out in full). This is from page 16 of the instructions.
