I really really think that proof per month is UNnecessary. If you have been attending school here, cumulative annual report cards for the years you've been here is sufficient to prove you have continuous residence. If you would like to submit additional info, a comp vaccination record or even medical records for visits for certain years may also be applicable. I have been here more more than a decade. I have sent school transcripts/reports cards for every year.
