Along with the actual EAD, I sent the document that came with the EAD, which asks you to keep it anyway in case you need to send your card back. I also sent a copy of my passport, consular ID card, and a copy of my birth certificate. I wrote a brief explanation of the mistake and how my name should read on the EAD. My mother's maiden name was on my EAD twice, once as my middle name and I do not have a middle name at all.
