sorry if this question has been asked already but on form i-765, for the question of "Which USCIS office?" do we put the address of the center that processed my application the first time? (Vermont Service center)?? And which date do we put? The receipt date or the notice date for when we got the 1-797 notice of action? Thanks for any help
