For 3 explain in the last section that you have originally entered without inspection, you have only ever entered without inspection once, and for your most recent arrival you got AP, traveled out, and re-entered using it.

Thanks for taking the time reply to my thread.

I just want to clarify my situation with #3 "Have you EVER entered the United States without being inspected and admitted or paroled?"

I remember at the time I was a kid (25+ years ago) and came with a guy as father, arrived on airplane originally and was "inspected and admitted before an immigration officer" at the airport, perhaps using another identity, he did not give me any copy for I-94, so I can't prove the entry with inspection.
However to answer the question, it would be a 'NO' and to explain "I was personally inspected, but not in my name, and I don't have I-94 to show."

I don't know the legal consequence, what's your take on it?
