When you go to the DMV bring your EAD, Passport, SSN card (SS printout may be acceptable), your DACA approval notice letter, 2 proofs of residence, money order or check. For ID, (it is $13.50) and DL ( it is $24.00 since you're only eligible for a 2 year photo), the completed DL 180 form ( http://www.dmv.state.pa.us/pdotforms...rms/dl-180.pdf ) and for State ID complete form dl-54a ( http://www.dmv.state.pa.us/pdotforms...rms/dl-54a.pdf ).

Oh and one more thing, don't forget to have the second page of the DL 180 form be completed by a licensed doctor or physician.
