"You must sign your application in the signature block if you are 14 years of age or older. Your parent or guardian may sing their name in the signature block if you are younger than 14. Your signature or a parent's or guardian's signature in the "Signature of person preparing form, if other than above" block is not a valid signature for this purpose. Please sign the application with the correct fees to the address listed on the bottom of the this page. "

I have email lockboxsupport@dhs.gov, and I will call them soon, but I want to know what you think.

My question is how many times did you sing your application? . I know there are two places. Are there more? What do you think?

I apologize for any possible spelling and grammar mistakes.

EVERYTHING UNDER PART 5. SPECIFICALLY PART 5 2. a AND OBVIOUSLY 2.B

IF YOU HOWEVER ENTERED INFORMATION ON PAGE 7 THEN YOU WOULD HAVE TO SING THERE, OTHERWISE NO.

THIS ISN'T A JOKE. READ DIRECTIONS.

I am assuming you're 14 years or older. If so,

1) Sign on Requestor's Certification Part 5, 2a.
2) Sign on Part 8 Additional information, 6.a.

Part 7 is only for person preparing the request other than yourself, for example, a lawyer. DO NOT SIGN there if you are filing it yourself.

part 7 not page 7.

also, you do not need to sign part 8 6.a if you did not entered any information. (which is PAGE 7)
