Here's what I wrote.

Dear Congressman/Congresswoman,

I am recipient of DACA, and currently renewing my work permit. I would like your utmost assistance to inquiry USCIS on my behalf. I have successfully applied and renewed my DACA/Work permit on timely in the past. But unfortunately, this time it's taking longer than expected approval time of 4 months. USCIS received my application on (enter date here) and tomorrow my work permit will expires.

(enter reason why you need EAD asap - lost of job, hardship supporting family, renew license, etc)

Here is my case number
(enter your case number here - i821d, i765)

(enter your thank you here)

Hope that helps,
all the best
