On Form i-765, question 15 (Current Immigration Status). I came here with a visitor visa in 1997. Do i put down Overstay Visitor or unlawful status: DACA?

Also on question 16, do we put ( C ) (33) ( ) or ( ) ( C ) (33) ?

for question 15 put unlawful status: DACA and 16 don't matter however you put it' s really up to you

I have a couple of addresses for the part 2 address part on form 1821D. I wanted to use part 7 additional info.

For page number I put 3, part 2. What do I put for "item number"?

If open the PDF on your computer browser, you'll see a drop down menu in which you see an array of options to select from. "Entry Without Inspection".

it is not EWI since she presented the card at entry. i did some research a few minutes ago..

"The Border Crossing Card (BCC) is both a BCC and a B1/B2 visitor’s visa. A BCC (also referred to as a DSP-150) may be issued as a laminated card..." "Border crossing card is also I-586. The status granted to the bearer is B-1/B-2, a visitor for business or pleasure."
