Yes, when I prepare packages I make copies of items that fit in several categories, and I put the codes at the bottom of each page of evidence, for example "1A" for the birth certificate.

From your list it looks like your bank statements end in 2009. That seems to leave a gap after the birth of your child until 2012. Perhaps I am misunderstanding your list.
