Just head over to ssa.gov and read up.
Everything you need is there, including the ss-5 form for you to fill out and print.

yes, right after i wrote that i received another notice saying USPS picked up the card and had a 3 letter tracking number. Good stuff.

No tracking number included with the email/text notices, but i suppose you could call and ask for one. Haven't received the actual letters that they mail out yet. ...

Long wait (4+ months) but I was also approved yesterday, Jan.22, after RFE!
Got the last e-mail today saying card was mailed.
