06/30/2016 Expiration date on I-821D

I downloaded the I-821D, Consideration of Deferred Action for Childhood Arrivals form from uscis.gov.

The top of the form says expires: 06/30/2016
Is this an issue? Not the latest form?

I did it on Thursday as well, and got it in the mail today. It should come way before 4 weeks.

Congratulations. I just started my journey... I received the EAD on Wednesday, and applied for the SSN on Thursday. I should get it in the mail within 2 weeks.

I was approved! I got no text messages or email.
Two letters came in the mail today!. Both were approved on April 18!
Was I suppose to get a physical card as well? other than...

I just called to inquire about my case for the first time. I had no luck!. The customer service rep gave me her generic talking points... pretty much repeating everything that I knew from reading...
