In the instruction, they said two separate checks, one for $380 and the other $85. It's in the same packet but it is two separate forms. I would do two money order, just to be safe but I'm not sure...

on the I-821D instruction form says you can use report cards. As long as it has the current date and his name. Or school transcripts that he can request from his school.

You should just put the address and explained the reason on the additional part so they know. I did that as well for the years I did not put since I was really young and moved around and couldnt...

I've already done that, thanks. I scanned both the new and old since my new one obviously doesn't have any stamps on them and the old one does. Someone also said some passport pictures as well.

Trust me, I've been reading a whole lot. My uncle whose done this before for his wife and is really familiar with the whole USCIS process..and he told me I didnt need the form I-821D since I came...
