No good reason, first they said that it" it was still being processed please allow more time " then the said " issues with processing that delayed my case" then they said that they had "issues...

This happened to me. I filled out both mine and my sisters app back in may of 2016 , our exp dates were both in early oct .

she was approved in early july . 8 months later and i'm still waiting....

I'm still waiting. 230 days since it was received. I submitted another service request for outside normal processing times. I received a response on Dec 23rd that it was " still pending adjudication...

It was the same way for me when filing. There are a couple of fields you must manually enter.

As for the form date as long as you got it from the link below its the correct one. Here are the notes...

That sounds like B.S . Fees did not go up until Dec 23rd. Anything postmarked before that date was still applicable for the old fees. How long have they had your application ? If they've had it...

Hard to deal with people who applied 5 months after you and have their card already. 207 days and pending for me ! congrats to all the recent applications though.
