When i try pulling it up from the USCIS website it just shows a black screen. All other forms are loading just fine for me. I've tried different browsers but i get the same problem.

Right click
this link and save it to your desktop. Then right click the file on your desktop and select the Open with Adobe Reader entry. You must use Adobe's Reader, otherwise the contents of the PDF file will not be loaded. If you don't have the software you can get it
here.
