In the first page just put without the maiden name. Just your first middle and last. Where it ask you to write any other names used I wrote : first and last. Then I used the last page as additional info and added my full name including maiden name. They will figure it out and issue your work permit and social security card with your full name as it appears in your birth certificate and other official id.
