Afaik,non-immigrant visa holders and Green card holders are the only ones obligated to carry paperwork to prove status at all times but US citizens are not!!!!

There are also US citizens who cannot prove they are US citizens as you said.I can definitely give some prominent current examples of this.I found 1 individual who didn't know they were a US citizen due to their parent serving in the vietnam war and gaining US citizenship through wartime.They came to the US on a visitor's visa and overstayed but the military is not giving access to records due to privacy regulations.The parent gained US citizenship before the child's birth.

There was another individual who came into the US on a green card as a minor but never renewed said green card due their parents Naturalizing before turning the age of 18.They never bothered to apply for a Certificate of Naturalization due to the CCA act because it is too expensive.

It maybe difficult to believe,but there are US citizens out there who unknowingly may believe they are actually undocumented just because of expiration of a document or don't have the necessary proof to prove otherwise.
