In Part 1, #10, anyone know what the city/town/village and country of birth is for Hong Kong? I know Hong Kong is a weird place, since I'm not sure if it's a state, or city, or separate from China or what not.

In Part 2, for present address, do we put the date from when we first lived there, or the date from the last renewal, since the instructions says to only list places since the last renewal. I forgot the actual date, so is an approximate date acceptable as well?

For travel information, #8, do I check no if I've never left the country at all? And is the passport information required? I believe my passport is expired, not sure if that will affect anything or not.

I will assume we leave Part 6 and 7 blank if we are preparing it ourselves.

For places that we don't need to fill out (like additional residences or travel, Translator/Interpreter info) do we leave it blank or put N/A as per the instructions on the instructions pdf?

Similar question to the one in I-821D, and that is the place of birth. How would I list Hong Kong as in the format provided (Town or City, State/Province, Country). If anyone knows, that would be great!

As for #12, is an approximate date ok as well here?

How did you guys fill this out? For my first renewal, the law office didn't ask me to do another one, so I assume they reused the one I did for my initial request. Just a bit of background, right now my brother (also DACA) and I are pretty much providing for the entire household. So maybe I just elaborate on that and list the income, expense, and assets, that will be fine?
