For form i765, I already have a work card that expires next year due to DACA, Am i supposed to file for a new one ? or a renewal ? And for the question that said If i ever been granted one and the location, do I put my local uscis or from where I mailed the form from daca application?

Sorry for so many questions. I need to file the application as soon as possible and at this moment can't afford the prices the lawyers were asking for. thank you
Ok so I'm currently in the same boat, filling out those same forms for my family members. My aunt is petitioning for my grandmother, and my uncle just got married and is petitioning his wife and stepson.
My answers will be based on the assumption that you have a legal entry, are going to be adjusting from within the US, and that you're doing concurrent filing.
1. You DO NOT need to file form I-765WS, that is only filed if you are requesting deferred action.
2. You DO need to file form I-693 (the medical exam). Form I-693 can either be included in the packet or wait until you get an RFE and send it in response. It is not a good idea to send it in-between these times because it will confuse USCIS and delay your case. Preferred method would be to send it in the package, all filed together.
3. The Stamped I-94 is sufficient, make sure to keep a copy and send one with your package! You can also send a copy of your passport, if it was also stamped or airline tickets (optional)
4. Yes, you leave the question about visa # and consulate blank. You can add notes on last page of form that you entered via visa waiver program (optional)
5. Yes, you can and should put your parent's info. Their undocumented status will not affect your application, nor will they be put on some target list.
6. WT-Visitor for pleasure- VWPP is correct. VWPP stand for Visa Waiver Pilot Program.
7. You can leave travel document section blank. Travel Document means a document used in place of a passport (ex. border crossing card)
8. Yes, you would fill out the local USCIS office. This is where you will have your interview.
9. Regarding the EAD (I-765), you should definitely file it as its FREE! Also it means you you will be able to update your SSN so it no longer says "Valid for work with DHS authorization" and your license will no longer be capped by your EAD expiration.
You would be filing for a NEW EAD document, so make sure to check the correct box. For the question about last EAD granted you would put what ever Service Center granted your EAD (ex Nebraska Service Center, Texas Service Center, California Service Center)
**Also don't forget the category for your new EAD will be (c) (9)
