For a waiver filing the spouse doesn't have to sign, so that part would be left blank. For her address you'd put down "Unknown, see attached explanation", and basically write a note that she moved out to fuck know's where and the last known address you have is where you lived together.

I actually don't know, but there's a reason why VAWA and Domestic Violence U-visas get handled by a special team in Vermont, those are people trained specifically in recognizing and reviewing domestic violence and domestic abuse claims. Generally you want at least a psychological evaluation from somebody that does them for VAWA applications, they'll know what questions to ask and put everything into a neat package. That being said, you can get approved without one. I submitted a pretty grisly self affidavit that went into a lot of NSFW detail and got approved. I was RFE'd but only for the bona fides, my letter convinced them from the get go.
