FYI, it does not need to be notarized.

It doesnt have to notarized it only has to be certified by any person that translates it. it can be you, or anyone that is fluent in both languages.
There is a sample around here in the forum of a translated mexican Birth certificate.

I saved the sample as a jpeg then opened it on paint and edited all the text with my info and then certify it.

hi guys Im new to this site & was wondering on a question on form I765WS # 2,3,4 im currently networking due to my pregnancy & my mom and bf pay for my expenses but its not alot.

You have to add up all of your expenses like food, clothing, housing, gas, insurance, utilities, it doesn't matter who pays for them, has to be a all of your expenses. Add them up and put them down in the form.

Your income is only what you personally, are earning, and not anyone else. Put that in the form.

If you have anything under your name like a house, a car, put that on the form.

Now, if you're income is less than what your expenses are, it is okay. The point of the form is to show that your economic necessity to justify your need for a work permit.

You do not have to itemize (give specific details) about your expenses.

if you have no income, or very little income with considerable expenses write: "I have no (or little) income and am meeting my expenses at a burden to others" and nothing else.

if you do have income and your expenses match closely to your income write: "I need work authorization so that I may continue to secure my income" and nothing else.

Most importantly, remember, they are asking about your personal income and expenses, not your family's.

I don't know what is my manner of entry, I came to USA with I94 and it said under section 212(d)(5)(A) and the purpose is public witness.

Question regarding the Additional Information section.
If the additional information is for "other names used", under additional information I would say Page 2, Part 1, numbers 12a, 12b, (and 12c if applicable) right? And then under the explanation field I would do:
12a. Johnson
12b. Diego

I am not familiar but I think you should write either "With inspection 212(d)(5)(A) Public Witness" if there is no visa number.

All I could find on 212 (d)(5)(a) was this http://www.uscis.gov/portal/site/usc...00082ca60aRCRD

and it indicates is Humanitarian Parole. But, not sure if you want to write that.

We came here 10 years ago on a visitor visa and haven't left since then. The problem is we don't have the I-94 form anymore which is where my question comes from. Under "Status at Entry" I put B2 Visitor Visa, but I'm really not sure about what to put for question 16. I feel like saying "No" would be incorrect because we DID have an I-94 at one point, but saying "Yes" requires me to enter the I-94 number in addition to the date my authorized stay expires. I have the old passport with the entry stamp on it which is one of the pieces of evidence I'm submitting along with everything else.
