You either have unending war in 7 countries, or you have healthcare.

Let there be competition among hospitals. Let more hospitals get built. In many areas the number of hospitals is capped, and to get a new one you have to get the other existing hospitals to agree. You have to get your enemy to let you build a new one.

Let the prices become transparent at each hospital. Many people dont even know how much they're gonna get charged until its too late and the bill is there. Most importantly when youre dying you cant gauge prices.
