You gotta think about it from your perspective first. Is it absolutely essential for you to send these form? Is it your only evidence during that year?
If not, and you got other easily verifiable documents during that year, why make yourself worried over nothing? Just don't send them.

I was looking through my application package that was sent (copies) and i realized that i DID NOT send proof that i was present in the UNITED STATES on June 15th 2012.

I have tax returns for the last 6 consecutive years my school transcripts (graduated high school 2009) and monthly bank statements from 2008 through present.

HERE IS WHERE I MESSED UP...
I sent the very first page of my monthly bank statements from 2008 to present, but i missed the one for JUNE (15)

The closest 1st page bank statements i sent were for:
APRIL24, 2012 - MAY 22, 2012
JUNE23, 2012 - JULY 22,2012

Now my question is: WILL I GET DENIED FOR MISSING THAT INFORMATION?
HOW CAN I SEND JUST THAT INFORMATION? (i do have that bank statement, and on top of that i also have a copy of my cellphone bill, which i did not send on my original application, dating June 14 2012 when i made my telephone payment.)
DOES ANY ONE KNOW WHAT I SHOULD DO?

PLEASE HELP -- I am WORRIED THEY WILL DENY MY APPLICATION.

If you are qualified, they probably won't deny you on the spot, they will probably ask for more evidences.

Okay, so I did a STUPID mistake.. I wrote the same country for both country of birth and country of residence!!! UGH! I called and they said they will fix it but does anyone else here made the same mistake? I know this will slow the process but I hope they will fix it before my biometrics appointment and I won't be rejected

I dont think you should worry, they will either send it back to you so you can correct it, or you can get that fixed at your Biometric check when they go over your info with you to make sure its all correct )

Sure! ) lol freaking out is normal especially knowing how important this really is to all of us. Hahaha right now Im still freaking out because I havent received any notifications since my biometrics x)

Okay. Thanks. I've got some documents for 2007 but wanted to be double sure. Then again don't want to get myself in trouble by being extra detailed.

I feel like I live in the Lounge.

if you are filing on the weekend, he would put what ever date was on friday or if you are filing today, you would put today's date if he went to school today...the last day he was present at school before filing

if he is in high school you would put: IN SCHOOL
