You only include Supplement A if you adjust under 245(i). Being a spouse of a USC who was paroled into the country you don't have to file under 245(i). Unauthorized employment is forgiven in cases of adjusting as an immediate relative of a USC (spouse, parent, or unmarried child under 21).

Update on my case
5/19 bio letter
6/7 biometrics
9/7 approval Ead letter
9/28 Ead received
03/22 Interview. Letter
04/22-04/25 had interview posted 3 weeks ago my experience so
Received Second biometrics letter 10 days after interview
I-130 has been approved and on the app it has changed
Went last week for biometrics rescan

Now I am waiting I noticed and word of advice to check biometrics for I-485 biometrics I looked at the one I had done and it was for I-751 and Daca and no I-485 code so they originally did not have my biometrics for the I-485 but had for the Ead. Now I am waiting anxiously I had no issues with earlier biometrics renewing Daca or others so not worried but I hate the wait. Wanted to share that way everyone checks their paper work

Update on my case

USC mom petitioned for me in November 2010. Filed for I-485 and I-765 in November 2016.
Interview in November 2017. At the time immigration officer said I am putting your case on hold pending a final background check.
No word about my case until December 2018 when I received notice that my case was denied due to an error on my part of filing I-485 too early. So they claimed. I was heartbroken but I was sure Immigration was wrong. I noticed on the denial notice they claimed my priority date as of 03/2011 and not 11/2010 as it said on the original receipt I received when my mother filed the petition.

USCIS was now claiming my priority date as 03/2011 due to an RFE we received when my mom filed the I-130 back in 2010. We were given till 03/2011 to reply to that RFE and we did reply on time. All other correspondence between them and us shows my priority date as 11/2010. Until that denial letter in December 2018.

I messaged Demise who kindly answered all my questions. I had to consult a lawyer to file an appeal to that denial. The lawyer agreed with me that USCIS was indeed wrong about the priority date on the case. We filed the I-290B in January of this year. In April I received notice my case was reopened. Today I got notice my case is approved and card is in production.

I did the 245i route as well. I had a ETA 750 Labor Certification and also had to show 12/20/00 prove of physical presence.
