write when you arrived and if you can't remember then ask your parent or just estimate the day and month and year. send what you have which is the visa and explain that you lost the i-94. this things happen. now make sure you have some pretty good evidence of coming here around the year you say you entered the US and include other papers of coming here before your 16 birthday and being here between 2007 and 2012.

This is referring to how you entered into the United States. Did you entered using a Visa? Did you cross the border without inspection?

If you crossed the border through the dessert, or through an official entry using other people's documents, and did not get caught, you can put the option of Enter Without Inspection.

My packet arrived Monday. How does it usually take to get an acceptances text ? Or rerouted message, I sent my packet from CA to AZ.

I payed the USCIS fee with a check and I was told that I would know my application was accepted when the check was cashed is this true ?

If they cashed your checked, that means that the forms were filled out correctly, as in no missing fields or signatures. They will start processing it, and you will receive a formal acceptance letter.

If you sent the G-1145 E-Notifications Form you should receive an email in about 3 days after you mail it, acknowledging that they received your application along with a receipt/tracking number.

You will then receive a formal written response in the mail saying basically the same thing as the email in about 7 days after.

My Status on Nov 2nd morning: CARD/DOCUMENT PRODUCTION and it said MY EAD HAS BEEN MAILED DIRECTLY TO MY ADDRESS. By Nov 2nd evening: POST DECISION ACTIVITY and it said They Mailed me an approval notice and that I have to follow instructions on that notice. So does that mean they mailed my EAD or no? has this happened to anyone?

I also lost my I 94 so I just put LOST where they ask for I94 number. However, I had entry and departure stamps so I sent them a copy of all the pages from my passport and my case got approved without a problem. Im sure most people lose their I94s so just write LOST and I'm sure there won't be any problems.
