YOu put what your VISA is called. If is H4. Entered with H-4 Visa.

I BELIEVE since your visa expired you put unlawful status. But on the sample application a lawyer gave me they put DACA requestor. I put DACA applicant. But the correct terms are EWI for entered without visa or unlawful status. Another lawyer that looked at mine put EWI, but I liked DACA applicant more.

- I thought maiden name refers to a woman's name before marriage.

Yes, it appears the same way as in my Birth Certificate.
It appears as Brown, James John, Lee.
When I applied to a university I used Brown, James John and that's how it is input in their system right now.

No problem. Remember to add (c)(33) to the I-765 in the part that ask for category. I think is question 16, sign and date your application in the correct place and label all your evidence. Look at the USCIS filing tips. Don't highlight anything on your applications.

What do you mean by "label all your evidence"?
Also in Form I-821, page 2, question 20, it says date of last attendance, graudation etc., I am currently in school so do I put that I went to school today or should I put the date of my high school graduation?

Are you in college?

I am in college too.

For the first Question: I put High School Graduate
Second Question: Since I am in College and it asks for City of school currently attending.-I put City of my current university.
Third Question: Where it asks for enrollment: I put ENROLLED since I am currently enrolled and currently attending.
Another Lawyer that looked at told me to put high school graduate, location of high school, and date of high school graduation. But I personally thought it was more logical to do what I did.

Well, I used an index and used page numbers for all my evidence. But this is what USCIS says
"Organize and label evidence by the guideline it meets."
But looking back, In any corner put your name, A# if you have one, and label evidence if its Identity Documents, Arrival Before 16, June 15, 2012 evidence, High School Graduation Evidence, and Continuous residence June 15, 2007-June 15, 2012
