My QUESTIONS:

I-821D:
On part 5:(Signature of person preparing this request, if other than the requestor)
I would leave it blank right? (bc i am the requestor)

On part 6: (signature of interpreter)
Same thing, interpreter was not used so do i leave it completely blank? ....I'm afraid they will think that i just forgot to fill it out or something.

On part 7: Additional information....blank also if no additional info needs to be given

**********All Blank. YES!

I-765
for number 9: Social security number (include all numbers you have ever used if any)
what do i write? none?--i don't have a "real one".

*****do not put fake ssn on the I-765 or I-821D just leave Blank.

for number 11: Have you ever before applied fir employment authorization from USCIS?
Should i say YES? I have a pending uvisa case and that has a I-765 along with it. case is still pending.
--- which office : Vermont Service Center (this is where all u visas are done)
---- Results ( granted/denied- attach all documentation)
Do I just put pending? and i don't have any documentation except my receipt number that i got when i did biometrics

Someone please help! Thanks!!!!

*****For this one put yes. Yes, Vermont Service Center. Results put pending. Do you have the I-797 and maybe the biometrics appoinmnet. Make a copy and send it in.

How is it really important to translate my birth certificate from Spanish to English?

How many of you didn't translate it?

Is there a sample I look at?

HELP

If your going to send it you have to translate it. But you can use other alternatives such as a passport in combination with other stuff on the list.

Maybe someone can chime in on this but as far as the directions from USCIS go your not allowed to translate your own. I had my friend proof read my translation and she signed off on the translation certification. All birth certificates are a little different but if you know know how to use word. What I did was I went section by section and created a text box for each section. So, it looked like a mirror image of birth certificate. Mine had some really fancy old school spanish so I just googled the words and most of the time I found they have been asked before.

1. Application # I-821D
2. Pg # 1
3. Q# 1.a
4. What is your question? In this part do we put our last name as it is in our Birth Certificate with both of our parent's last names or just the last name I use now which is my dad's.
Thank you.

You are supposed to put what appears on your birth certificate and then on the second page under other names used you put just your first and last name and other names used.

Thank you!
And can we staple the forms so they won't be all over the place?

You can staple them but I recommend you don't because they are scanned. Try to keep it organized with paper clips for now. Number all your evidence and label it to the corresponding requirement. So, it is obvious.
