when filling out form SS-5 when applying for SS card do we have to put down our parents info?

I sent my application on 2/20 and got it back a week later with the form I-797C. I hate how it doesn't specify what is wrong. At first I thought it was the money order since it was for $466. So I send a new one for $465 and send it 2/28. And again got rejected. I assume its the money order since its staple to the I-797C notice. Is the money order suppose to be blank??

The last two times were filled from me to US Department of Homeland Security.

Did you make sure all your papers are signed correctly? Did you miss any questions?

Did you include the I-765ws, I-765 and I-821D?

Do I attach money order with paper clipl or what? My documents are ready and hope punched at the top. I don't want to hole punch the money order.

Yes, you can paperclip it. Don't staple or hole punch it.

How should I package all of this? is there a standard envelope i need to get

Can I send a check, if so who should i pay it too?

Do i need others papers to be translated beside the birth certificate?
I got my vaccination records from my country not sure if i need them to be translated too

On Form I 765 WS. I have been working here without papers should i put zer0 on #2,3 and 4?
Do I need to put details on #5 of why i have zeros?

You can send it in any envelope but there is an express address and standard mail address http://www.uscis.gov/portal/site/usc...0045f3d6a1RCRD

You can send a check and it goes to US deparment of homeland security it is in the directions.

Not sure why you are using your vaccination record? I'm not sure about your last question. Maybe someone who worked under the table can answer that.
