3. For the USCIS office, do I put "Vermont Service Center" ? (that's what it shows on my approval notice letter)

4. According to this thread and checklist (http://dreamact.info/forum/showthread.php?t=68487), I need a copy of my I-94? Is that even necessary? And also passport photos? What do I really need to send?
