New version of I-765 form question !

Hi! Everyone

Has USCIS updated a new version of I-765 form? It look a little bit different to me.

I have a question regarding the form , on item 10 " do you want SSA to issue you a social security card " ?

Should I answer " no " ?

I already applied for SSN after my initial DACA approval in 2013,
I still have the card now.

2nd question is " how to find out which USCIS office approved my EAD "? My receipt number starts with IOE.

We are under C33 category, do I need to answer question in item 23b ( have you ever been arrested for or convicted of any crime ?)

1. Item 10, if you already have a SSN which you said you did, answer "no"

IOE is USCIS ELIS (e-file)

3. I don't believe you have to put anything for 23b since that's a different category. I don't believe I answered 23b on my application.

*Note, i'm not an immigration lawyer but this is my third renewal

The receipt number for previous renewal of DACA and EAD all starts with IOE .
I noticed that on the bottom left of my DACA approval notice , it says “ Nebraska service Center “

Should I put “ USCIS ELIS or Nebraska service center “?

I put Nebraska on mine. I don't think where you got approved should be a major issue for them. I had an IOE receipt.
