notarized affidavits

Do we get the people to write them, then take them to the notary public or do they take the letter themselves to the notary public?

I believe the person has to sign the letter in front of the notary public and the notary public will write down his or her information down and archive it.

Does anyone have a generic one that has the key details necessary to make it valid? Can it just be a basic letter stating that the person vouches for having lived here from a certain date?
