need help with question 15, 19, on form I-821D

on 15 i put entry without inspection since I came here illegaly (under a car seat), and for 19 it ask for name city and state of school currently attending or where education received,, I was in high school but I droped out and later took my GED exam which I pass, should I put the name of my high school or the nane of the place where I took my GED? I didnt take GED classes I just took the test quick.. and this a stupid question but where it says to state my full last name, should I put both my last names (Torres Berume) or just one?

Yeah, I think the popular term was EWI entered with out inspection, and about the GED, I would think you would and add documents to back that up. Do you have any paper evidence for your GED? And Yes, where it states to put your full name, PUT YOUR FULL NAME. Otherwise, its going to be confusing when they look at any federal (both foreign and domestic) you submit and have different names, these centers are not very smart about this. Only in the area it asks for FULL NAME. If it asks for first name, put your first name, if it ask for last name, put your last name.

yes I have my GED certificate but in question 19 it asks where I received education, I received education in High school should I put the name of it even If I didnt graduate? or should I put where I took my GED test (I didnt enroll for ged classes) i just took the test.. and yes on form I-821D it asks for Family Name(last name) should I put both?

You probably have to put the place, city and state where your GED was issued, not your high school, unless the high school issued the GED
